STEP 2: Register problem recognition into an organizational ledger
Once an air that lets negativity be voiced exists, the next step is a mechanism that lists and tracks problems organizationally. This is the build-out of Level -1 (problem recognition). Organizations with heavy trust debt can use the response-loop mechanism stood up in Phase 3 of trust repair as the seed of STEP 2’s Issue Register; you don’t need to rebuild it from scratch. Let it evolve naturally.

Every problem identification flows into one place. Tooling (Notion / Confluence / Linear / Jira) doesn’t matter; what does is meeting the following:

The “my judgment and reasoning” field is the load-bearing one. Without it the manager can’t evaluate the reasoning, and falls back into micromanagement. For the writer, articulating the judgment is itself thinking practice.

This is the stage where you take the “compensating individual” from the parent article and move their knowledge into the organization, while respecting them. The 42% of organizational knowledge that is individual-specific in the Panopto survey18 starts breaking down here. Forcing the compensating individual to write alone increases their load and breeds resentment. Do it in pairs:
- Another person plays the questioner, the original holder answers
- The questioner is the one who writes (not the holder)
- 30-minute weekly sessions
- The completed document is shared with the team; the holder retains reviewer rights
This is the same pattern as Stripe14’s documentation culture and the technical-writing practice of “interview-based documentation.” Distributing the writing responsibility is what makes it sustainable. Putting a new hire in the questioner role is also a counter to curse of knowledge19 — they’ll catch the missing premises in real time. Cognitive training for the writer side, explicit “intended reader” and “prerequisite knowledge” fields, and glossary linking are unpacked in “Curse of knowledge: why expert-written docs don’t reach new hires, and how to fix it”.
6-C. Document “questions you’ve answered recently”
In 1on1s, ask “did you answer the same question multiple times this week?” Take 30 minutes to turn it into a FAQ / wiki entry. This alone produces 10–20 documents a month, naturally. Pro tip: write in two layers — a “30-second answer” and a “follow this link for detail.”
6-D. Don’t make the compensating individual the villain (handling status anxiety)
Mishandle this and the compensating individual stops cooperating. Same as the response to the E status-anxiety pattern:
- Make explicit that “single-point dependency is an organizational problem, not a personal one”
- Wire “contribution to organizational knowledge” explicitly into evaluation: leverage indicators like “people taught,” “successors developed,” “reference frequency”
- After the documentation is in place, give them a new senior role (mentor / architect / principal-equivalent)
- In 1on1, surface conversationally that “being irreplaceable is exhausting for you too”

7-C. Feed the negative signal to AI too (the parent article’s logic, implemented)
A backlog of postmortems is exactly the resource for telling AI “don’t do this.” Anthropic15’s framing of Context Engineering as “minimal high-signal” makes past failure patterns extremely high-signal.
If you only feed positive material, AI returns proposals detached from reality. AI is also a counterpart that can convert negativity into motivation, in the Defensive Pessimism8 sense. Include in the context you give AI:
- Reasons for past failures and walk-aways
- Current weaknesses and known issues
- User complaints
- Areas where you lose to competition
Caveat: failure cases often contain personally identifying or sensitive information. The realistic operation is to maintain summarized versions that have been anonymized and reduced to key points before AI input.

Cross-cutting threats in detail: 12 patterns
On top of the seven steps, several structural patterns will attack from the side during implementation. Focus on the ones flagged in Diagnostic 2 and weave responses into your operation. Each pattern deserves its own article; here we just lay out symptom, mechanism, and direction of response. Each heading links to the standalone article with diagnostic checklist, anti-pattern table, and implementation steps.
A. Frozen middle: middle managers monopolizing information as a power source
Symptom: leadership is forward-leaning, the front line wants to move. But the initiative gets absorbed and stopped at the middle layer. Retros become “we’re too busy this week, push to next week” right before they start, ADR templates get rejected with “doesn’t fit our team,” registration into the Issue Register gets blocked with “I’ll explain it myself.”
Mechanism: their power source is “being the only ones with cross-organizational context.” Documentation and sharing dissolve that monopoly directly. Kotter22 in his change-management writing repeatedly notes that what kills change is not the opponents but the silently immobile middle. Quy Huy in his 2001 HBR piece23 inverted the framing: middle managers, properly engaged, are the strongest change agents available.
Response: present a new power source for middle managers (evaluation axis = team growth / number of strong people developed, not information monopoly) / design distribution so content flows through them (don’t strip the hub function — change the substance) / surface their career concerns directly in 1on1s / leadership commits to role changes for those who can’t adapt.
B. Sponsor disappearance risk
Symptom: the executive flag-bearer (CEO / CTO / CHRO / etc.) leaves, transfers, or gets reorganized out. Within six months, the initiative is hollow.
Mechanism: a single-sponsor design is fragile. Kotter22’s 8-step latter half (short wins and embedding into culture) is precisely the response, but it gets neglected at implementation time.
Response: embed the initiative into evaluation, budget lines, and OKRs so it survives a personnel change / use a multi-sponsor team (CEO + CTO + division head, etc.) / build it into public commitments (external announcements, hiring pitches) so the reputational cost of retreat goes up / a succession protocol at sponsor change: the next person in role automatically inherits sponsorship.
C. The “AI makes documentation unnecessary” myth
Symptom: “AI will figure it out, we don’t have to write it down” / “ask AI” spreads from leadership to the front line, and the budget and time for context maintenance gets cut.
Mechanism: AI cannot encode what is not encoded. Anthropic15’s framing of Context Engineering — “curate and maintain the optimal token set” — presupposes there is something to curate. AI becomes the excuse for reducing context supply.
Response: codify as an executive-level principle that AI input is a derivative of existing documents, not something AI generates from scratch / when reviewing AI output, count edits attributable to context shortfall as evidence the myth needs to be retired / make “AI is the mirror of an organization’s context supply capability” an internal training artifact.
D. Documentation theater / dead documents
Symptom: 10,000 wiki pages, more than half of them more than 18 months old. Decisions get made on stale information, and bad calls follow. The official line is “we have a documentation culture,” but real dependence remains personal.
Mechanism: there is incentive to write, but no incentive to age out. Unread documents have no one to notice they’re old. Compliance- or audit-driven documents have no readers and are dead from birth.
Response: every document has a mandatory owner and “next review” date (un-reviewed → auto-stale) / a deprecation sprint twice a year: consolidate, delete, reconcile / make access logs visible: low-read documents become deprecation candidates / don’t introduce volume KPIs at all (see next pattern).
E. Status anxiety in tacit-knowledge holders
Symptom: in STEP 6 pair documentation, the compensating individual performs cooperation without delivering. They show up to interviews but withhold the core. The resulting documents are “the bare minimum.”
Mechanism: their evaluation, status, and employment stability are anchored in being “the irreplaceable specialist.” Documentation directly threatens that. The argument “we value you, please write it” is structurally contradictory.
Response: wire “contribution to organizational knowledge” explicitly into evaluation (leverage indicators like people taught, successors developed, reference frequency) / give them a new senior role (mentor / architect / principal-equivalent) after documentation is in place / surface conversationally in 1on1s that being irreplaceable is exhausting for them too / leadership publicly states “having more people in this position makes the organization stronger.”
F. Goodhart’s Law on context KPIs
Symptom: the moment you make “ADR count,” “wiki pages,” “postmortems run” into KPIs, volume increases and substance hollows out. The “one decision per file” rule breaks and “vanity ADRs to inflate the count” appear.
Mechanism: the modern formulation of Goodhart’s Law (Marilyn Strathern, 1997)24: when a measure becomes a target, it ceases to be a good measure. Context maintenance is fundamentally a question of quality, but KPIs slide to volume easily.
Response: treat KPIs as health indicators, not targets / look at “times referenced,” “times reused” instead of volume / aggregate indicators are fine subjective; rushing to objectify invites number gaming / hold a quarterly meeting that questions the KPIs themselves.
G. Tool sprawl and the collapse of the single source of truth
Symptom: documents on the same topic scattered across Notion / Confluence / Slack / Google Drive / GitHub wiki / Quip. Search returns nothing useful, and people fall back to “ask that person.”
Mechanism: tools accumulate by department, era, acquisition, and individual leader preference. The decision to consolidate sits in no one’s mandate and stays unmade.
Response: decide single source of truth per topic (code-related decisions in GitHub, HR in Confluence, strategy in Notion, etc.) / cross-tool search (Glean / Notion AI / internal search) / enforce deprecation when introducing a new tool: one in, one out / continuously surface “couldn’t find what I needed last week” in 1on1s.
H. The “culture-fit” hiring filter
Symptom: even after 4-phase trust repair starts improving psychological safety, hiring keeps screening out candidates who would dissent on “fit.” Three years in, the monoculture hasn’t budged.
Mechanism: Lauren Rivera’s 2012 American Sociological Review paper25 showed that elite-firm hiring is dominated by “cultural matching” — the cultural similarity between evaluator and candidate — to a degree that overrides ability evaluation. “Culture fit” easily slides into “dissenter exclusion.”
Response: replace “culture fit” with “culture add” (evaluate the diversity the candidate brings) / put people who left over past negative observations into the interview loop (consistent with Phase 2 of trust repair) / structure-and-articulate “the discomfort I felt in the interview” (treat “doesn’t feel like a fit” as a banned phrase).
I. Curse of knowledge: writers are too expert
Symptom: documents written by the compensating individual leave new hires unable to follow anything. Premises are stripped, terms aren’t defined, no diagrams.
Mechanism: Camerer, Loewenstein, and Weber’s 1989 Journal of Political Economy paper on the “curse of knowledge”19: once you know something, you cannot reconstruct what it is like not to know it. Documents written by experts carry this bias structurally.
Response: mandate STEP 6-B pair documentation: separate writer from questioner / appoint new hires as document reviewers while they’re learning (they catch the missing premises in real time) / require every document to have explicit “intended reader” and “prerequisite knowledge” fields / require domain terms to be linked to a glossary.
J. Founder / power-distance organization
Symptom: the founder is the de facto context engine for the organization. Trying to write down explicit ADRs or strategy maps gets blocked with “I make the decisions, no need to write it down.” High-power-distance cultures (Hofstede; including Japan and East Asia) show the same pattern.
Mechanism: when a single person at the top is the sole judge, documentation means distribution of decision authority. That reads as a challenge to the power structure itself.
Response: cast the founder / top as “context editor” and put them on the writing side (visualize the power, don’t take it) / frame documentation as “decision succession”: “so the organization can keep moving on the same axes after you’re gone” / reframe ADR as “learning record” rather than “decision record” / sequence: start with the strategy map; individual ADRs come later.
K. Legal / compliance-driven suppression of documentation
Symptom: in regulated industries (finance, healthcare, legal), legal cites “discovery risk” to suppress documentation. “Don’t write it” / “don’t even keep it in Slack” — and organizational knowledge stays inside individuals’ heads.
Mechanism: legal risk and knowledge accumulation trade off. Legal optimizes for short-term risk avoidance; the long-term cost of not accumulating knowledge is invisible.
Response: filter what to write: judgment logic (preserve) vs. case-specific judgments (preserve carefully) / co-design with legal a legal-reviewed format: a template for “writing in a way that’s safe to keep” / measure both the risk of oral-only knowledge (misunderstanding, single-point dependency) and the risk of documentation, then compare / separate internal-only retro documents from externally disclosable summaries.
L. High attrition or heavy contractor reliance, making accumulation impossible
Symptom: annual attrition above 30%, high share of contractors, frequent reorgs — and whoever wrote the document is gone in six months. Documentation gets stale by the time the next person arrives, and no one is the owner.
Mechanism: accumulation requires continuity, and the organization is structurally non-continuous. Attrition itself is a separate problem, but as a precondition for context maintenance it cannot be ignored.
Response: anchor document ownership to role, not person — succession follows the position / build “take ownership of one orphaned document” into the first-week onboarding tasks / address attrition itself in parallel (compensation, career path). Context maintenance alone cannot solve this region.
Measuring overall progress
When measuring overall health across the seven steps, aggregating individual KPIs too aggressively introduces noise (and invites F Goodhart-style runaway). Three aggregate indicators are proposed. Compensation Dependency
A subjective quarterly read of “what share of organizational knowledge is concentrated in three or fewer specific people?” Starting from the Panopto 42%18, track the decline.
Negativity reception
Anonymous quarterly survey: “In the past three months, did you raise a negative observation you noticed to the organization?” “How was it handled?” Track the product of raising rate × reception rate.
What happens when you skip steps
“Run all steps in order” is the ideal. In practice, the temptation to skip is strong:

Limits of this guide
First, even if you follow this guide perfectly, it doesn’t work unless executives thaw the root of organizational silence. In organizations where STEP 1 stays performative, every step from STEP 2 ossifies into ritual.
Second, context maintenance does not guarantee AI ROI. The 70% people-and-process number from BCG26 and the 80% organizational-rewiring number from McKinsey27 are correlations, not sufficient conditions. Without the other necessary conditions — AI strategy, data preparation, model selection, security architecture — even a fully built-out context layer produces limited returns.
Third, scale matters. The guide fits 10–100 person organizations naturally. At thousands of people, “one company-wide handbook” stops being realistic, and you need division- and region-specific operational design. This guide doesn’t extend that far.
Fourth, measurement is hard. The aggregate indicators are partly subjective and don’t translate well across organizations. Use them to track the same organization over time.
Even with these limits, the author’s view is that the implementation in this guide carries enough investment justification on its own — both as the precondition for AI investment ROI and as a set of byproducts independent of AI (faster onboarding, lower knowledge loss, better remote readiness).

The piling-on of failures, traps, and pitfalls isn’t to push the reader into despair — it’s to present them as raw Obstacle material in the Defensive Pessimism / WOOP sense, in a form that converts into motivation. There is a legitimate reason “negative thinking” gets a bad reputation: single negatives that stop at problem identification drain the organization’s energy without moving it forward. That is exactly why what should be cultivated, when building a culture that welcomes negativity, is the paired negative — the mode of thought that bundles confrontation with a direction of response. The Obstacle and Plan of WOOP, the negativity that Defensive Pessimism converts into motivation, every section of this article being written in “symptom + mechanism + response” — all of it is the same paired-negative structure. And completing the paired negative requires both a sender skill (attaching a proposed direction) and a receiver skill (listening through to that proposed direction), plus attention to generational and individual sensitivity differences. If, having read this far, you feel “I think we can do this,” that is itself evidence that your organization has the soil to drive positive emotion. Start at STEP 1. If asked “what do I do on Monday?” — start with an executive publicly owning one of their own judgment errors at all-hands. The signal that thaws organizational silence comes before any technical move; the meaning of the order condenses to that point.
Conversely, if it feels “too heavy to even pick up,” your trust debt is probably heavy. Begin with Phase 1 of trust repair (acknowledging the past concretely). An executive or manager, in first person, names a specific event — that, alone, is the starting point.
“Facing problems” is not the opposite of hope. It is a means to hope. An organization that can look its problems in the face is in a state where positive emotion can be generated. An organization that has held up under the weight of this article already holds half of the path forward.
